Application & Required Forms
Below are step-by-step instructions for completing all required items for the Mississippi Wing Cadet Retreat. Please READ ALL INSTRUCTIONS before completing an application.
Date: Friday-Sunday 13-15 October 2023
Location: Camp Yocona 878 Boy Scout Rd, Randolph, MS 38864
Drop Off: Friday 13 October from 1700-1900 (5-7 pm CST)
CAC Drop Off: Friday 13 October from 1600-1700 (4-5 pm CST)
Pick up (all): Sunday 15 October at 1300 (1:00 pm)
Step 1 Registration Form:
The registration form is the first step in securing a position at the MSWG Cadet Retreat. This is a Google Form. You will need to fill out the forms below (CAPF 60-81; CAPF 160,161,163; and the photo release form) and upload them to this Google Form when you fill it out.

Step 2 Required Forms:
Upload the completed forms below to the Google Form when you fill it out to attend. There is a place to upload docs.
*Please make sure that the documents are labeled this way: Last name, First name Form name
*This will ensure that we receive all the necessary documents required for your cadet to attend the Retreat.
Application for Special Activity
(CAPF 60-81, 2 pages, signed by all parents/legal guardians)
- CAP Member Health History Form
(CAPF 160, 2 pages, signed by parent/guardian if applicant is under 18 years old)
- Emergency Information
(CAPF 161, 1 page, signed by parent/guardian if applicant is under 18 years old)
- Permission for Provision of Minor Cadet Over-The-Counter Medication
(CAPF 163, 1 page, signed by parent/guardian and required if applicant is under 18 years old)
- Copy of applicant’s Insurance Card (copy of front and back, if applicable)
Camp Yocona info and directions
Step 3 Payment:
The cost is $40 per cadet and MUST be paid at arrival check in. Cash or Check accepted at drop off. Please make checks payable to MSWG Civil Air Patrol (put cadet name on memo line)
-If you are riding in a CAP van please ensure that you have your payment with you.
-Payment MUST be made at arrival or you will be sent home.

Please do not call the MS Wing general phone number with questions or send messages to other MSWG email addresses or via Facebook. They are not part of the Cadet program staff and do not have current information.
